Case Manager

Part time position to work primarily with elderly population in rehabilitation setting. Must possess a keen attention to detail and possess excellent written and oral communication skills. Some regional travel is required to provide in-home services. Prior exp. working with the elderly or the disabled population, particularly the visually impaired, is a plus. Associates Degree in Human Services or related field is req’d. Must have strong computer skills that include MS Office applications. Equal employment opportunity employer.
